Ms. Dionne Leverette, RN, BSN, MHA-INF, ACM, ACHE is a nursing leader with 30 years of nursing experience in various fields. During her career, she has been privileged to work with healthcare professionals and patients across many different populations in several states. She has been contracted with the Millennium Physicians Group as the Senior Vice President for Population since March of 2022. In this role, Dionne is accountable for developing and implementing system-wide care coordination management strategies with a patient-centered approach across the continuum.

Dionne is also responsible for ensuring all patients receive a safe transition of care regardless of location. She has spent the last 15 years of her career developing social programs addressing healthcare disparities and creating equal and equitable access to patient care regardless of the patient’s ability to pay. Healthcare access and equity are two major pillars of her work.

Dionne graduated from the University of Phoenix, with a Master of Health Administration & Informatics in 2010. Her undergraduate work was completed at the University of Virginia School of Nursing where she received a Bachelor of Science in Nursing in 1993. She has been a Certified Case Manage with National Board for Case Management (AMCA/ANCC) since 2009 and is a current member of the Florida Case Management Association. She is also a current member of the American Council of Health Care Executives (ACHE).
